Documents and forms

Documents is the unified workspace for files and generated clinical forms. For staff, it opens with client folders or a client list; select a client to work with that chart's documents.

Upload and review documents

Inside a client folder you can search by title, description, or file name and filter by status or category. Choose Upload document to add a file. New items can be marked Pending review; staff can mark them Reviewed and choose whether to share them with the client. Administrators can delete an item, which archives it for retention and removes it from normal lists.

Clients can upload their own files. Their uploads appear in the same workflow for staff review. A document or upload does not independently grant strict chart access.

Document access is care-team scoped in every clinic, even when the clinic uses open charts for other clinical modules. A practitioner must be the client's primary or secondary practitioner, or hold a valid audited break-glass grant, to list, open, download, or upload documents for that client. Clinic admins retain tenant-wide document access.

Templates and forms

When Templates & Forms is enabled:

  • Templates lets staff create and maintain reusable form templates.
  • Community lets staff browse shared templates and add a copy to their own library.
  • Generate from template creates a client-specific document from a saved template.
  • Pending items shows forms or consents awaiting client completion or signature.

Generated documents remain part of the client's Documents folder, including their status and sharing controls.

Clinical notes

Staff can write notes for a client in the document workspace. Clients see a read-only Notes from your care team section; they cannot edit staff notes. Use document visibility controls for files, and the notes workflow for clinical observations rather than uploading an unrelated placeholder file.
