Medical information

Open Profile → Medical Info to maintain the structured baseline attached to your account in the current provider workspace. This information is separate from uploaded files, generated forms, and notes in Documents & records.

Fields you can maintain

The current form provides:

• blood type, including Unknown;
• primary language and known allergies;
• checkboxes for diabetes, hypertension, asthma, heart disease, stroke history, and Alzheimer's disease;
• emergency contact name, relationship, and phone number;
• organ-donor status; and
• additional comments, up to 1,000 characters.

Select Save Clinical Details after reviewing every changed field. The record is workspace-specific: updating it at one clinic does not change your record with another provider.

The form requires the provider's Medical records module. If it fails to load or save, do not assume the update was recorded; retry or contact the provider.
