Managing your care team
In workspaces that expose client-managed care teams, open Profile to choose from the clinic's public practitioners. Your care team determines ordinary practitioner chart access when strict chart access is enabled.
What the primary practitioner means
Your primary practitioner is the main practitioner assigned to your care in that workspace. If your team has no primary practitioner, the first public practitioner you add is set as primary. Additional practitioners are added as secondary care-team members.
Primary and secondary members both receive the treatment-relationship access defined by the workspace's strict-access policy. A practitioner cannot be both primary and secondary at the same time.
Adding a public practitioner
In the care-team section of Profile:
- open Add a practitioner;
- choose an available public practitioner; and
- select Set as primary or Add to care team, depending on whether a primary practitioner already exists.
Only practitioners who currently allow public discovery can be added through the client self-service selector. Clinic staff can manage assignments under their own authorization rules.
Changing the primary practitioner
Select Make primary beside a secondary member. The selected practitioner becomes primary and the previous primary is demoted to the secondary team; the backend keeps the two groups consistent automatically.
Removing a practitioner
Select Remove beside the practitioner. The confirmation dialog explains that their normal chart access will end and checks for upcoming appointments with that practitioner. If appointments are found, you can choose whether to cancel them before completing the removal.
Removing a practitioner does not delete historical appointments or clinical records. You can add an eligible public practitioner again later.
Chart access and emergencies
Under strict chart access, a practitioner can open your chart through exactly one of these sources:
- they are your primary practitioner;
- they are a secondary care-team member; or
- they hold an unexpired break-glass grant.
Appointments, messages, prescriptions, tests, and uploads do not independently grant access. Public booking and staff-created appointments automatically add the assigned practitioner as primary when the team has no primary, or as a secondary member otherwise. Starting a new conversation with a practitioner also adds them as a secondary member when they are not already on the team. That resulting care-team membership is what grants ordinary access.
Documents follow this care-team rule in every workspace, including clinics that otherwise use open charts. Break-glass is an emergency path: the practitioner must record a reason, the grant expires, and clinic admins can review the audit entry.
